College students with irritable bowel syndrome who exercised regularly reported substantially less severe gut symptoms than their sedentary classmates. This finding comes from a cross-sectional study of 872 students in China, led by researchers Yuqian Sui, Ping Li, and Xianfeng Yue. The work provides new, quantifiable evidence for a gut-brain connection, suggesting that the mental health benefits of movement are a significant part of why physical activity helps IBS.
How the Study Measured Activity, Mood, and Gut Symptoms
The research team surveyed students from three universities in Shandong Province. They defined regular physical activity (RPA) as engaging in moderate-to-vigorous exercise for at least 30 minutes, three or more times per week. Students reported their activity levels and completed three standardized questionnaires.
The Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) measured psychological state. Gut symptoms were evaluated using two tools: the disease-specific Irritable Bowel Syndrome Severity Scoring System (IBS-SSS) and the broader Gastrointestinal Symptom Rating Scale (GSRS). This two-pronged approach gave a comprehensive view of IBS-related discomfort. The researchers then used group comparisons, multiple regression, and formal mediation analysis to unpack the relationships between activity, mood, and symptoms.
Active Students Had Lower IBS Severity Scores
The difference between active and inactive groups was clear. Students in the regular physical activity group had significantly lower scores on both the IBS-SSS and the GSRS. This means they experienced less frequent and less intense abdominal pain, bloating, and bowel habit disruption.
Multiple linear regression models, which control for other variables, confirmed a strong independent association. Regular physical activity was linked to a 28.7-point decrease on the IBS-SSS and a 3.5-point decrease on the GSRS. “These observations indicate that RPA is inversely associated with IBS-related symptom severity,” the authors wrote. Simultaneously, the analysis showed that higher SAS and SDS scores—indicating more anxiety and depression—were consistently associated with higher, worse scores on both gut symptom scales.
Depression Accounts for a Share of the Benefit
The most important finding lies in connecting these two results. The statistical mediation analysis tested whether the beneficial effect of exercise on IBS symptoms operated, at least in part, by reducing depressive symptoms. The result was significant.
The researchers found an estimated indirect association between physical activity and IBS-SSS scores through depressive symptoms. Quantitatively, depressive symptoms statistically accounted for 19.43% of the total association. In simpler terms, nearly one-fifth of the gut symptom improvement linked to exercise was explained by its positive impact on mood. Anxiety was also measured but did not show a significant mediating effect in this model, pointing to a specific role for depression in this gut-brain pathway.

The 19% mediation figure is not an absolute value, but it underscores that mental health is a major component. It argues against a purely gastrointestinal treatment model. Effective management should integrate physical and psychological strategies.
